Registration Process

All first-time patients must complete the registration process before availing hospital services. Registration can be completed either by visiting the OP Registration Counter or by booking an appointment over the phone.

• Direct Registration: Visit the OP Registration Counter during working hours.
• Telephone Appointment: Call 0497 276 2500 to schedule your visit.
• Our registration staff will guide you through the required registration and appointment procedures.

New Patient Registration

• Complete the registration form accurately with your personal details.
• Please provide your current mobile number for future communication and appointment updates.
• A unique Hospital Number (UHID) / Patient Identification Card will be issued upon registration.
• Proceed to the concerned outpatient clinic with your registration receipt, which contains your hospital number, appointment date, and reporting time.
• Keep your Hospital Number Card safely for all future visits.
• Provide complete and accurate medical history, current symptoms, medications, and lifestyle information to help your doctor provide the best possible care.
• If admission is required, your consultant will issue an admission order.

Follow-up Visits

Patients returning for follow-up consultation should bring their Hospital Number (UHID) / Patient Identification Card to the OP Registration Counter.

• No new registration form is required for follow-up visits.
• Please carry your Hospital Number Card during every hospital visit.
• Your OP file may require renewal as per the hospital's registration policy.

Advance Appointments

To avoid waiting time, patients are encouraged to schedule appointments in advance. Our Appointment Desk is available every day to assist you.
